Insurance and Accepted Plans
We work with a wide range of insurance providers to make dental care as accessible as possible. We currently accept Aetna, Ameritas, Assurant, Blue Cross Blue Shield, Cigna, Delta Dental, DHA, GEHA, Guardian, Humana, MetLife, Principal, United Concordia, United Healthcare, and VA benefits. If you are unsure whether your plan is accepted or how your benefits apply to a specific treatment, our front desk team is happy to help you sort through the details before your appointment.
We recommend bringing your insurance card to each visit so we can process your coverage accurately and minimize any delays.
Financing Options
We understand that dental care is an investment, and we want it to be within reach for every patient and family we serve. In addition to accepting major credit cards including Visa, Mastercard, Discover, and American Express, we also offer financing through CareCredit as well as an in-house financing option. If cost is a concern, please speak with our team and we will work with you to find a payment plan that fits your situation.
Office Hours
We offer flexible scheduling to accommodate busy families and working adults. Our hours are as follows:
- Monday and Tuesday, 8 a.m. to 6 p.m.
- Thursday, 8 a.m. to 5 p.m.
- Friday, 8 a.m. to 3 p.m.
- Wednesday appointments are available by request
We are closed on weekends and on major holidays, including Memorial Day, the Fourth of July, Labor Day, Thanksgiving, Christmas, and New Year’s Day.
Online Patient Forms
To save you time at check-in, we offer new patient forms online. Completing your paperwork in advance means less time in the waiting room and a faster path to meeting Dr. Collier. You can find the forms on this site. If you have any trouble accessing them or prefer to complete them in the office, just let our team know and we will have everything ready for you when you arrive.

You are not alone, and we are prepared to help. Please let us know before your appointment if anxiety is a concern. We offer sedation options including nitrous oxide and oral sedation, and our team will always move at a pace that keeps you comfortable.
Do you offer cosmetic services?
We do. Our cosmetic offerings include teeth whitening, veneers, dental bonding, and smile makeovers, among others. If you are interested in improving the appearance of your smile, ask about a cosmetic consultation.
What if I need to cancel or reschedule?
